Lift Permit
This permit must be posted/available at the site for the duration of the work.  This permit is valid for one shift, not to exceed 12 hours. The completed Lift Plan and TSA must be attached to this permit for the lift to proceed.  Return the signed, completed permit to IISN HSSE.
