This LOTO (lockout/tagout) Energy Isolation Procedure establishes the minimum requirements for the control of hazardous energy during the maintenance, modification or servicing of equipment when the unexpected energizing, start-up, or release of stored energy could cause injuries.
The fundamental minimum requirement of LOTO is that each individual servicing a piece of equipment shall personally lockout & tagout every source of energy associated with the equipment. This procedure discusses the practical ways this requirement is met and the various exceptions at this site.
Listed below are Appendices included in the LOTO Procedure:
Appendix A: Equipment Isolation List
Appendix B: Line Breaking Procedure
Appendix C: Standard LOTO Tag for CAF
Appendix D: Lockout/Tagout Program Review
Appendix E: Types of Energy
Appendix F: Examples of Roles in Type II LOTO
On pressurized systems where proper mechanical Lock Out Tag Out (LOTO), per site standard, and zero energy has not been achieved, refer to Engineering Document Number NAP-4.5-ENG-0400 "CAF Procedure for Isolation of Hazardous Energy When Mechanically Breaking Containment". This document provides a procedure and direction for circumstances where a zero energy state cannot be achieved and mechanically breaking containment may proceed provided proper PPE and adequate ventilation are in place.
